St. Michael's Hospice and Sauk Centre Home Care provide a team of professionals including registered nurses, physical therapists, occupational therapists, social worker, home health aides, homemakers, and volunteers to meet the needs of clients. We care for recovering, disabled, or chronically ill people who need medical treatment and/or assistance with activities of daily living. Services are paid for through a variety of payment resources. Referrals and consults are available by calling us at 352-2221 or having your social worker, discharge planner, physician, or family member to so for you. We cover a 30 mile radius of Sauk Centre.
