St. Michael's Hospital, Nursing Home and Hospice/Homecare offer licensed social workers to assist with your needs. Social workers can assist you with resources to better your hospital stay and your discharge needs. Social workers can also assist with continued support and advocacy in long-term care. Care and support is offered for patients and their families through end-of-life provided by St. Michael's Hospice.
